2.4 NDIS Participant Information: if you have a NDIS plan, we may access some authorised plan information through the NDIS participant portal.

2.5 Information from other Service Providers or Professionals: we may access personal and/or plan information about you from plan managers, the NDIA, support coordinators and other health professionals or service providers that you authorise.

 

3. Use and Disclosure of Personal Information

The Form Movement’s primary purpose for collecting personal information is to provide services to its clients and participants, and it will not sell, trade, reuse or otherwise disclose any personal information to a third party without prior consent.

Non-personal identifiable information, including but not limited to browser type, version and language, operating system, pages viewed while browsing the website, page access times and referring website address might – as of above – be collected when visiting our website, and this is for visitor traffic purposes only.

 

4. Information Security

The Form Movement is committed to ensuring that all personal information is held securely and will take regular and reasonable steps to protect personal information from misuse, unauthorised access/use, modification, interference or disclosure and loss.

 

5. Accessing Personal Information

The Form Movement will disclose information and documents it holds upon request from the individual, with exception to the limitations imposed by privacy legislation. For security reasons, The Form Movement might require the individual’s request in writing. A request to provide information will be handled within reasonable time (30 days). The individual can request personal information to be updated/corrected by contacting The Form Movement directly.

The Form Movement reserves the right to modify its Privacy Policy at any time in accordance with this provision. Upon changes to this Privacy Policy, we will update the privacy page on our website at https://www.theformmovement.com.au/ and changes will come into effect immediately from the time of such publication. Your continued use of our services and website following such changes will constitute your acceptance of any changes. If you disagree with our revised Privacy Policy, you may terminate your agreement with The Form Movement in accordance with the terms of our agreement.
